We canceled this year's program in Toronto due to the outbreak of COVID-19. Our first priority is the health, safety, and the security of students, patients, and all the people in society. This page reports on the outcomes of preparatory lectures, workshops, seminars, and online learning related to this program.
2019年度の派遣(2020年2月、3月)は、新型コロナウイルス(COVID-19)の感染が拡大する状況で、参加者や派遣先の皆様の健康と安全を第一に考え、中止いたしました。ここでは、TCTPに関連する派遣以外での1年間の学びを報告します。
SickKids 特別講義1(招聘薬剤師 Sabrina Boodhan)SickKids Special Lecture 1 (Sabrina Boodhan, Pharmacist)

- Pharmacy awareness campaignsが日本にはないもので、とても楽しそうでした。日本にもこのようなキャンペーンがあれば、薬剤師の職域が広がり、より多くの人に薬剤師という職業を認知していただけると思います。Pharmacy awareness campaigns are something we do not have in Japan, and they looked like great fun. If Japan had campaigns like this, the scope of pharmacy practice would widen and more people would recognize the profession.
- 日本でもカナダでも薬剤師のあり方は同じなんだなと感じた。I felt that what it means to be a pharmacist is the same in Japan and Canada.
- 自分が薬剤師として働く際にテクニシャン制度がかなり影響してくると思います。今回テクニシャンについて知れてよかったです。The pharmacy technician system will have a big influence on how I work as a pharmacist. I am glad I learned about technicians.

- 今日の講義では、子供は治験例数の数も少なく、オリジナルで薬物治療のモデリングをして、その患者、また同様の疾患を持つ患者の治療に役立てているとそのような分野が発展している話を聞いて、大変興味深く、視野が広がった気がします。Today’s lecture explained that because there are few clinical trials in children, original modelling of drug therapy is used to treat the patient and others with the same disease, and that this field is developing. It was fascinating and broadened my horizons.
- SickKidsがどのような組織か、海外の研究機関と大学との関係性について知れたことは良かった。研究するにあたり、なぜそれを調べるのか、なぜ重要なのかという要因で“impact”が重要であることを聞いて、新しいことを発見するためには革新的な発想や方法が必要であると感じた。It was good to learn what kind of organization SickKids is and how research institutes and universities relate overseas. Hearing that “impact” — why you study something and why it matters — is key in research, I felt that discovering something new requires innovative ideas and methods.

- Bench to bedsideという言葉に興味を持った。臨床の場から検体をとって実験できるのは面白そうだった。The phrase “bench to bedside” caught my interest. Being able to take samples from the clinic and experiment on them sounded fascinating.
- トロントでの薬学研究について少し知ることができとても有意義な時間になった。将来について悩むことが多くなってきたこの時期に聞くことができとても嬉しかった。It was a very worthwhile time in which I learned a little about pharmaceutical research in Toronto. I was glad to hear it at a time when I have been worrying more and more about my future.
- とても分かりやすく噛み砕いて話して下さり、わかりやすかったです。TNFについての実験がおもしろいなと思いました。The talk was broken down very clearly and easy to understand. I found the experiments on TNF interesting.
- 自身が行なっていた研究がTranslational medicineだったことを知り、患者に寄り添った研究を行えていたことを嬉しく思った。I realized that the research I had been doing was translational medicine, and was glad to know that my research was close to patients.
- 治療法が見つかっていない患者さんの遺伝情報の変異から新たな治療法を見つけるなど、最先端の研究や医療がされていることを知りました。I learned about cutting-edge research and care, such as finding new treatments from genetic variants in patients who have no treatment yet.

- 小児の特徴について、身体や臓器の機能面における特徴は学校での講義等を通じて学んでいたが、今回の講義ではそれに加えて精神面における特徴も知ることが出来た。小児に薬物を使用する際には、薬物そのものの効果や製剤的な工夫(プロドラッグ、腸溶錠など)ももちろん重要だが、味や剤形など、飲みやすさや小児が嫌悪感を抱かないかどうかがさらに重要である。実習では小児患者と接することがほぼ無かったために、新規の薬物療法開始や薬剤変更について考える際にも、味や剤形(注射は嫌がられるために避けるなど)については全くといっていいほど考慮していなかった。そのため、今回の講演を聞いて特に小児に対する薬物治療では、そういった点に関する知識も必要で、十分に考慮して薬剤決定をしなければならないと気付くことが出来た。I had learned about the physical and organ-function characteristics of children in classes, but this lecture also taught me about their psychological characteristics. When using drugs in children, the effect of the drug itself and formulation techniques (prodrugs, enteric-coated tablets and so on) are of course important, but taste and dosage form — how easy the medicine is to take and whether the child will dislike it — matter even more. I had almost no contact with paediatric patients during my clerkship, so when thinking about starting or changing therapy I had hardly considered taste or dosage form (for example avoiding injections because children hate them). This lecture made me realize that in paediatric drug therapy such knowledge is essential and must be taken fully into account when choosing a drug.
- 実務実習では小児患者と関わる機会がなく小児の薬物療法について深く学ぶようになったのは本留学プログラムへの参加を決めてからであるが、勉強するほどに成人との違いや小児特有の問題点、課題が明らかになっていき、今まで知らなかったことや自分の中に無かった考え方を学ぶことが出来た。適応のある薬が限られていることや、使用できる剤形が限られているという状況は適切な薬物治療を行う上で大きな障壁となるが、小児に対してはそれらの障壁を乗り越えて治療を行わなければならない。トロント小児病院での実習や薬剤師になってからの臨床での学び、経験を積み重ねて、小児に適した薬物治療の実施をサポートできる薬剤師になりたいと思った。I had no contact with paediatric patients in my clerkship, and only started studying paediatric drug therapy seriously after deciding to join this program; the more I studied, the clearer the differences from adults and the problems specific to children became, and I learned things and ways of thinking I had never had before. The limited number of approved drugs and available dosage forms is a major barrier to appropriate therapy, yet for children we must overcome those barriers and treat them. Through the training at SickKids and the clinical learning and experience I will gain as a pharmacist, I want to become a pharmacist who can support drug therapy suited to children.
- 添付文書で「小児や妊婦・授乳婦に対する安全性は示されていない。」という記載をよく見かけるが、小児に対する適応外処方がおよそ64%にも及ぶことは知らなかった。日本の国民皆保険のような医療体制では、治験が最新の治療を受けることができる機会ととらえられる可能性が低いことや安全性の懸念からも治験を受けるケースは少ないとのことだった。適応外処方では、原則として保険は適用されず、自由診療扱いとなるが、十分に科学的根拠が認められる場合や、高度医療に用いる一部の新薬などに限り、例外的に保険適用・保険診療との併用が認められる。しかし、適応外処方による医療事故が発生した場合の責任の所在は不明であり、医療者側の立場を守るためにも適応拡大は進めていくべきだと強く感じた。そのためには、治験数を増やすことよりも現在行われている適応外処方に対する1つ1つの症例についてエビデンスとなるように日本全体で協力してデータを集め、適応拡大を促進していけるように努めることが必要であると感じた。Package inserts often say “safety in children, pregnant and breastfeeding women has not been established,” but I did not know that about 64% of prescriptions for children are off-label. Under a system like Japan’s universal health insurance, clinical trials are rarely seen as a chance to receive the latest treatment, and safety concerns also mean few children enter trials. Off-label prescribing is in principle not covered by insurance, though exceptions are made where there is sufficient scientific evidence or for some new drugs in advanced care. But when an adverse event occurs with off-label use, responsibility is unclear, so I strongly felt that expanding approved indications should be pursued, also to protect health care providers. Rather than simply increasing the number of trials, Japan as a whole should collect data on each case of current off-label use so that it becomes evidence, and work to promote expanded indications.
- 今回の特別講演を通して、小児の薬物治療は添付文書通りにいかないことが多いため、小児の特性をしっかり理解した上で、様々なエビデンス、経験が重要になってくる分野であると感じた。講演で学んだ内容をしっかり復習し、さらにもっと知識をつけて準備をし、これから参加するトロント留学プログラムでより多くのことを学ぶことができるよう頑張りたいと感じた。This lecture made me realize that paediatric drug therapy often cannot follow the package insert, so it is a field where evidence and experience become important on top of a solid understanding of children’s characteristics. I want to review what I learned, build more knowledge and prepare, so that I can learn as much as possible in the Toronto program.
- 「小児は成人の小型ではない」と言われる通り、小児の身体機能は成人のものと比較して異なり、未成熟な場合が多い。各薬剤の小児に対する影響は臨床試験が行われていることが少なく、情報が少ないことは実習を通して感じていた。しかし情報が少ない分、安全性の担保されない治療が行われていることを本講義で改めて痛感した。小児の投与量を決定する上で様々な計算式が存在するが、これはあくまでも推測の域を出ないことを肝に銘じておきたいと思う。一方でこれだけ成人と異なり情報の少ない小児薬物療法はわからない部分が多い分、自分の興味が惹かれる領域のようにも感じた。As the saying goes, “children are not small adults”: their body functions differ from adults’ and are often immature. I had felt during my clerkship that few clinical trials are done in children and information is scarce, but this lecture brought home that, precisely because information is scarce, treatment is given without guaranteed safety. Various formulas exist for calculating paediatric doses, but I want to remember that they are only estimates. At the same time, paediatric drug therapy, so different from adults and with so many unknowns, struck me as a field that draws my interest.
- 薬物に対して様々なことがわかってきている中で、小児領域に関する情報がここまで少ないのは正直驚きだった。米国や欧州では情報の少なさに対する対策を行なっていることも初めて知ったし、日本が遅れている現状を改めて感じてしまった。自分が薬剤師として臨床現場に出る頃には、小児医薬品開発ネットワークのような情報収集システムがより発展していることを考えると、もっと勉強しなければならないと思うのと同時に、そこか楽しみのようなものも感じた。トロントでは日本とは医薬品に対する制度などが異なり、患者さんの医薬品に対する考え方も異なる。日本の薬剤師も将来的には現在の外国薬剤師のような形に変わっていくのではないかと個人的には考えているので、将来像を見るという意味でもトロント研修では日本との違いに注視して2週間の生活を無駄なく送っていきたいと考える。With so much now known about drugs, I was honestly surprised at how little information there is in paediatrics. I learned for the first time that the US and Europe have measures to address this, and felt again that Japan is behind. By the time I am working in clinical practice, information systems such as paediatric drug development networks will be more developed, which makes me feel I must study more — and also excited. In Toronto the regulatory system and patients’ attitudes to medicines differ from Japan. I personally think Japanese pharmacists will eventually move toward the model of pharmacists abroad, so during the two weeks in Toronto I want to pay close attention to the differences from Japan and make the most of every day.
- 小児の薬物動態に関して、よく言われる言葉がある。それは、「小児は成人の小型ではない」である。成人と比較して、小児は臓器などが未熟であり、脂肪量や水分量が成人とは異なる。脂肪が一番未熟で、脳に脂肪が多い事から脂溶性薬物が脳にたまりやすいことがあげられる。また、体内水分量が多いことから、水溶性薬物が薄まりやすいことがあげられる。ADMEでいうと、吸収の面では新生児・乳児は成人より遅く、最高血中濃度到達時間が遅れる。しかし、新生児・乳児は皮膚が薄いなどから経皮吸収は成人よりも良好である。分布の面では、小児は脂肪量が少なく細胞外液量が多いことから水溶性薬物の分布容積が成人より大きくなる。代謝の面では、新生児・乳児では肝臓での代謝が遅い。排泄の面では、新生児・有事ではグルクロン酸抱合が未熟なため、肝臓からの排泄が遅れるなどがある。 小児は特有の事柄が多く、勉強することがたくさんあると感じた。トロント小児病院は世界でも有数の小児病院なので、いろいろなことを吸収して、自分の将来に活かしたと思った。There is a well-known saying about paediatric pharmacokinetics: “children are not small adults.” Compared with adults, children’s organs are immature and their fat and water content differ. Because the brain has a high fat content, lipophilic drugs tend to accumulate there; because total body water is high, hydrophilic drugs are more easily diluted. In terms of ADME, absorption in neonates and infants is slower than in adults, delaying the time to peak concentration, though percutaneous absorption is better because the skin is thin. In distribution, children have less fat and more extracellular fluid, so the volume of distribution of hydrophilic drugs is larger. In metabolism, hepatic metabolism is slow in neonates and infants. In excretion, glucuronidation is immature in neonates and infants, delaying hepatic elimination. Children have many special features and there is a great deal to study. SickKids is one of the world’s leading children’s hospitals, and I want to absorb as much as I can and use it in my future.

- 留学の準備のためにたくさんの先生方にお世話になりました。講義や準備期間を通し、苦手な英語に触れる機会が増えたこと、海外の医療について学べたことなどは私にとってプラスになったと思います。実際にプログラムに参加できなかったことは残念でしたが、自分の活動の幅を広げることができ、挑戦してよかったと思います。ありがとうございました。Many teachers helped us prepare for the program. Through the lectures and the preparation period I had more contact with English, which I am not good at, and learned about health care abroad, which I think was a plus for me. It was a pity that we could not actually take part in the program, but I broadened my range of activities and I am glad I took on the challenge. Thank you.
- 事前講義やCV作成、オンラインインタビューなどを通して、通常の学科の勉強や実習では出来ない貴重な経験をすることが出来ました。小児医療や海外の医学・薬学分野について学ぶと共に英語での書類作成や面談も行い、悪戦苦闘する事もありましたが仲間や先生方に助けられて楽しく学び続けられました。今回現地での研修が中止となってしまったのはとても残念ではありますが、事前準備を通して多くの事を学んだり自分で考える機会を得られて、とても有意義な時間を過ごすことが出来ました。この経験を活かし、将来臨床の場で薬剤師として医療に貢献していきたいと思います。Through the preparatory lectures, writing a CV and the online interview, I had valuable experiences that ordinary classes and clerkships cannot give. I learned about paediatric medicine and medicine and pharmacy abroad, and also prepared documents and had interviews in English; it was a struggle at times, but with the help of friends and teachers I kept enjoying learning. It is very disappointing that the training in Toronto was cancelled, but through the preparation I learned a lot and had many chances to think for myself, and it was a very meaningful time. I want to use this experience to contribute to health care as a clinical pharmacist.
- 私は小児医療に興味があり、プログラムに参加することにしました。留学準備のみとはなりましたが、医療英語や海外・国内の医療体制、小児医療に対する薬剤師の関わり方について学び、考える良い機会になりました。自分の将来の薬剤師像を改めて考える上でも、貴重な経験になったと思います。留学にあたり、トロントの先生方とのSkype面談など初めてのことも多く不安もありましたが、先生方や一緒に参加する友人達のサポートもあり、積極的に活動に参加することができました。ありがとうございました。I joined the program because I am interested in paediatric medicine. Although it ended up being only the preparation, it was a good opportunity to learn and think about medical English, health care systems in Japan and abroad, and how pharmacists are involved in paediatric care. It was also a valuable experience for rethinking the kind of pharmacist I want to be. There were many first-time experiences such as the Skype interview with the Toronto staff, and I was anxious, but with the support of the teachers and the friends taking part with me I was able to participate actively. Thank you.
- 今回はコロナウイルスの影響でトロント研修が中止になり非常に悔しい思いでした。トロントに行くにあたり多くのことを学びました。実務実習を終えた上で海外の医療に目を向けることは、自分の視野がとても広がることにつながりました。日本での薬剤師像が全てではなく、より広い視野を持って、この先どうなりそうなのかを考えた上で、将来薬剤師として活躍していきたいと感じました。I was very frustrated that the Toronto training was cancelled because of the coronavirus. I learned a great deal in preparing to go. Looking at health care abroad after finishing my clerkship broadened my horizons enormously. The image of the pharmacist in Japan is not everything; I want to keep a wider view, think about where things are heading, and work as a pharmacist with that in mind.
